Job Summary
Direct Support Professionals are responsible for advocating and supporting people with disabilities in a dignified and respectful manner with the development of skills necessary to become more independent in their daily lives. This position involves empowering the people we support to make their own choices and to lead self-directed lives, providing the support necessary for people to develop the skills to find and maintain employment, and assisting person's supported in creating connections and sustaining relationships in the communities in which they live.
Responsibilities
Encourage and empower the person supported to live more independently.
Foster a meaningful relationship between the person supported and their community.
Assist people with disabilities with their daily living and independence skills/personal care (may include grooming, bathing, feeding).
Utilize Person Centered Plans to assess a person supported needs.
Administer medication(s) and complete appropriate documentation.
Accompany the person supported to and from appointments and activities if needed.
Transport and support out in the community: outings/field trips, medical appointments, and shopping
Assist with daily living skills including meal preparation and assist with medication administration
Provide guidance on a range of developmental goals from community participation, household budgeting, exercise, nutrition, and supporting vocational responsibilities
Support persons served with living skills such as personal hygiene, grooming, and/or bathing
Qualifications
Must be at least 18 years of age, or older.
Current driver's license, car registration, and auto insurance
Reliable transportation
Work Authorization (required)
Other licensure or certification where required by regulatory authority
Excellent communication skills with an ability to establish rapport with team members and those we support
Strong organizational abilities to ensure staffing and schedules are maintained
Effectively read, write, and communicate verbally in English, and in the member’s first language if English is not fluent in English.
Able to read and understand instructions, perform record-keeping, and write reports as required.
Successfully pass criminal background checks
Must have a minimum of a year experience providing care to people living with an intellectual and/or developmental disability.
Education
Must have a high school diploma, GED, or equivalent.

Job Summary
The Licensed Practical Nurse (LPN) provides skilled nursing care and oversight to individuals receiving home and community-based services. The LPN ensures that all care is person-centered, respects the dignity and independence of the person served, and is delivered in accordance with the physician’s orders and the individual's plan of care.
Responsibilities
Provide direct nursing care in accordance with each individual’s plan of care.
Administer medications, enteral feedings, oxygen therapy, and ostomy care as prescribed, and assess and document individual responses.
Monitor and document health status, nursing interventions, and progress toward health goals; report significant changes to the physician, RN, case manager, and family/guardian.
Accurately document all nursing care, communication with health professionals, and updates to care plans.
Assist in the development, implementation, evaluation, and revision of plans of care in collaboration with the RN and interdisciplinary team.
Implement nursing interventions and preventive procedures to meet identified health needs.
Provide health-related education to the person supported and their family or support team, based on individual needs.
Coordinate and/or order necessary medications, supplies, and nutritional items to ensure continuity of care.
Ensure all services follow provider orders and agency protocols.
Observe and maintain infection control and aseptic techniques.
Attend and participate in staff meetings, required trainings, and agency in-services.
Maintain accurate training records and participate in ongoing professional development.
Ensure safe, quality, and compliant nursing services in all care environments.
Perform additional duties as assigned by supervising RN or agency leadership.
Qualifications
Graduate of an accredited school of practical or vocational nursing.
Current, unrestricted LPN license in the State of Tennessee.
Minimum of one (1) year of nursing experience required; experience with individuals with intellectual and developmental disabilities (IDD) preferred.
Current CPR certification (professional level).
Valid Tennessee driver’s license, acceptable driving record, and proof of automobile liability insurance.
Ability to perform venipuncture, lift and transfer individuals as needed, and manage the physical demands of in-home care.
Strong documentation and communication skills.
Ability to support individuals in diverse home environments and uphold their rights, preferences, and dignity.
