Community Living Supports (CLS)
Our Community Living Supports (CLS) services provide personalized, person-centered assistance that helps individuals with intellectual and developmental disabilities build independence, develop daily living skills, and actively participate in their communities. We focus on dignity, choice, and empowering each person to reach their individual goals.
Nursing Care
Our skilled nursing services provide personalized healthcare support, including medication management, health monitoring, wound care, and assistance with ongoing medical needs. We focus on safety, comfort, and quality of life.
Personal Assistance
We provide individualized support with daily living activities such as hygiene, mobility, and routine care, ensuring each person’s comfort, dignity, and health needs are met.
Respite
Respite care offers short-term relief for caregivers while ensuring individuals continue to receive safe, attentive, and compassionate support in their absence.
Supportive Home Care
Our supportive home care services provide compassionate, personalized assistance to help individuals remain safe, comfortable, and independent at home. Support may include personal care, daily routines, meal preparation, household tasks, and other everyday needs while promoting dignity, choice, and quality of life.
Family Model
Our Family Model services provide individuals with a safe, supportive, and family-centered living environment. Individuals receive personalized support with daily living, community involvement, and skill development while building meaningful relationships and greater independence.
